A valid "User Name" and "Password" are required to gain access to the on-line publications and to subscribe to product E-Mail Alerts. Registrants can create their own user name and password by
Form field followed by an asterisk ( * ) are required and cannot be blank. If any of these field are left blank, an error message will appear informing you that there is required information missing. The form will then be re-displayed with the erroneous field names highlighted. Enter the required information and click on the "Submit Registration" button to continue with registration.
This section contains information that will allow us to contact you via regular mail or by phone/Fax as necessary. NOTE: You must check the "Add to Mail List box to insure your subscription to E-Mail alerts is activated.
Check the box or boxes as appropriate for each model helicopter and information service you wish to receive alerts/notices for.

Registration Form Field Descriptions
User Name* - (Required field) Identifies the user for login purposes. Does not have to be the Customer I.D. number. No spaces allowed .
Password * - (Required field) Authorizes the user's access. Passwords may be changes by the user on the Update Registration form. The user must be logged in to update their registration information. No spaces allowed .
Confirm Password* - (Password - Required field) - When logging-in, the user must confirm the password by retyping the password in this field. No spaces allowed .
First Name* - (Required Field) Registered user/customer first name.
Last Name* - (Required Field) Registered user/customer last/surname name.
E-Mail* - (Required Field) Registered users e-mail address. This is the address that MDHI will use to send E-Mail Alerts and any other information concerning this users registration information.
Add to Mail List - (Required field) Adds registering users to e-mail list. This is required to allow for notification of registration. Important: This box must be checked.
Web Site - (Optional) Registering user's website URL (address).
Company Name or "Self" * - (Required field) Registering user's company name or enter "Self" for individual use..
Phone * - (Required field) Registering user's telephone number.
Fax - (Optional) Registering user's fax number.
Address * - (Required field) Registering user's address.
City * - (Required field) Registering user's city.
State (Province) * - (Required field) Registering user's state or province etc.
Zip (Postal Code) * - (Required field) Registering user's zip code or postal code.
Country * - (Required field) Registering user's Country

Model 369A (OH-6A) - (Optional) Check this box to subscribe to product and service information for the 369A (OH-6A) model helicopter.
Model 369H (MD500C) - (Optional) Check this box to subscribe to product and service information for the 369H (MD 500C) model helicopter.
Model 369D (MD500D) - (Optional) Check this box to subscribe to product and service information for the MD 500D model helicopter.
Model 369E (MD500E) - (Optional) Check this box to subscribe to product and service information for the MD 500E model helicopter.
Model 500N (MD520N) - (Optional) Check this box to subscribe to product and service information for the MD 520N model helicopter.
Model 600N (MD600N) - (Optional) Check this box to subscribe to product and service information for the MD 600N model helicopter.
Model 900N (MD Explorer - 900/902) - (Optional) Check this box to subscribe to product and service information for the MD Explorer series of helicopters (models 900 and 902).
Subscriber I.D. Number - (Optional) Enter your Customer I.D. number to subscribe to on-line access to publications. Your Customer I.D. number can be found on the Mailing labels of any technical publications sent to you. You can also obtain you customer I.D. number by contacting Danny Shadaram (480) 346-6373 or (800) 388-3378 at the Publications Order desk.
